　　通常推荐信表格常有一些常见问题需要申请者回答，问题如下：

　　How well do you know the student academically？

　　在学业上，您对该生了解多少？

　　In what years did you teach the student？

　　您是在哪几个年纪的时候教过该生？

　　How large is the class？

　　班上一共有多少人？

　　What courses？

　　在哪些课程中，您曾给予该生指导？

　　Brily describe your course. It is especially helpful to know what texts are used and if the students are grouped by ability？

　　请简单描述一下您所教授的课程，使用什么教材？学生是否是依照程度分班？

　　How accurately does the student read and understand what he or she has read？

　　根据你的观察，该生的理解和阅读能力如何？

　　How well does the student write in comparison with other students whom you have taught？ please be specific about areas of strength and weakness.

　　和其他学生相比，该生学习能力如何？请清楚告诉我们他在写作上的强项和弱点。

　　How well does the student accept advice or criticism？

　　对于师长的批评和指导，该生能虚心接受吗？

　　What are the first three words that come to mind to describe this student？

　　当您想到该生时，你首先想到那些词汇？

　　please comment on this student's character， citizenship， and contributions to your community.

　　该生对于您的学校有什么贡献？

　　please add any additional information that will give us a more complete picture of the student.

　　还有哪些信息，能帮助我们更进一步了解该生？

　　pS.有的学校还会提供“个人推荐信表格”（ personal Recommendation) 让推荐填写，表格中常见的问题如下：

　　plesase describe your relationship with the candidate. How long have you known him or her？

　　请描述您和该是怎么认识的？你和他认识多久了？

　　plesase indicate the level of talent and commitment he or she demonstrates.

　　请描述他或者她 在某个领域的天资和成就。

　　plesase comment on his or her personal qualities. What makes this young person special？

　　请描述他的人格特质，是什么样的人格特质，让该生显得如此特别？
